Fatal work accident
A mechanic lost his life on 4 May 1990 during work on completing the 2/4 W module in the Ekofisk Complex, when a pig was to be pumped through a water pipeline from 2/4 K.
While some small leaks in the system were being repaired, a bang was heard and a powerful water jet shot out of the pig receiver. This hit the mechanic, who was killed instantly.
The report into the accident identified a combination of causes. Its most serious conclusion was that the factors in the work procedures which could create a hazard were not identified and corrected by Phillips’ internal systems.[REMOVE]Fotnote: Ekofisk, no 3, 1990: 15.
